Please note: If you created a patient portal account prior to May 31, 2017, you must create a new account following the instructions below.

How to access our Patient Portal.

  1. Click here to go to the Patient Portal.
  2. To create a NEW account, you will click “Create an account” on the right side of screen.
  3. Create an account by entering the following information:
    • Username: Create a unique username.
    • Practice ID: Enter DQZFDZ
    • Password: Create a password.
      • Password requirements:
        • At least 1 uppercase character (A-Z)
        • At least 1 lowercase character (a-z)
        • At least 1 numeric digit (0-9)
        • At least 1 special character (~!@#$%^&)*;?=_)
    • Confirm Password
    • First Name
    • Last Name
    • Date of Birth
    • Zip Code
    • Security Code: You will receive a security code via email after scheduling your first appointment. Call (877) 544-3880 if you need assistance.
  4. Click “Create Account”.
  5. Select three (3) Security Questions. You may not use the same question more than once.
  6. You will then be taken into your account in the Patient Portal.
  7. Once in the portal, click “myChart” in the top right corner.
  8. Then, click “Forms” to complete the New Patient paperwork.
    • NOTE: Completed sections will have a green check mark. Incomplete sections will have an orange check mark.
  9. Once you have fully completed the paperwork, you will see a message stating “Completed: 1. HIPAA Consent, Financial Consent, Patient Medical History”.
